SCOPE

Kalafhi Day Hospital invites suitably qualified companies that are interested in providing the following products and services to submit their proposals:

COMPLIANCE REQUIREMENTS

Expression of Interest submissions should be accompanied by the following documentation:

1. Company Profile:
   • Overview of the company, including background, years in operation, core competencies, physical address, and banking details.
   • Certificate of Incorporation (for companies only).
   • Certified copy of trading license.
   • Valid Tax Clearance Certificate issued by the Botswana Unified Revenue Service or tax pin with certificate number.
   • Certified copy of identity document(s) for shareholder(s). If a company is a shareholder (Shareholder Company), identity documents of shareholders of the Shareholder Company must be submitted.
2. Cover Letter:
   • Detailing the type of service/products the company is applying for (maximum of three categories).
3. Capability Statement:
   • Details on the ability to supply and support the required goods and services, including any relevant certifications.
4. Relevant Experience:
   • Examples of past projects or contracts of a similar nature, including references.
5. CVs of Key Staff:
   • Providing details on the qualifications and experience of key personnel.
6. Compliance and Certifications:
   • Proof of compliance with industry standards and regulatory requirements.
7. Contact Information:
   • Primary contact details for the individual responsible for the EOI submission.
8. Certified Copy of Identity Document:
   • For individuals.

EVALUATION CRITERIA

The expression of interest will be checked for completeness and compliance with requirements of invitation for expression of interest.

The selection will be made based on the following:
